I NEED TO SUBMIT A CLAIM AGAINST AN ESCROW ACCOUNT–IS THERE A FORM FOR THIS?

Check with the ‘entity (title company, etc) that is holding that escrow & see what they require. Also, it might be spelled out in the escrow agreement you entered into when that escrow was established.
